De Quervain’s Tendonitis / Wrist Tenosynovitis

What is De Quervain’s tenosynovitis?

De Quervain’s tenosynovitis is a type of tendonitis that involves swelling around wrist tendons that lead to your thumb. Overuse (repetitive motions), hormone fluctuations such as pregnancy, and swelling related to systemic diseases like diabetes or rheumatoid arthritis may contribute to the painful condition. Direct trauma to the thumb or wrist may also lead to De Quervain’s tenosynovitis. 

What are the symptoms of De Quervain’s tenosynovitis?

The symptoms of De Quervain’s tenosynovitis can include:

  • Pain around the bottom of your thumb
  • Swelling at the bottom of your thumb
  • Difficulty grasping or pinching with your thumb

Ganglion Cyst

What is a ganglion cyst?

A ganglion cyst is a benign fluid-filled bump that often develops on the wrist or fingers, around joints, or tendon sheaths. The tissue balloons upward and fills with a thick clear gel-like liquid to form the bump.

What are the symptoms of a ganglion cyst?

The most obvious sign of a ganglion cyst is a bump on the wrist or finger that may change in size. Due to the pressure caused by fluids, some patients may develop pain. However, some patients do not have any discomfort.

Distal Radius Fracture

What is distal radius fracture?

Distal radius fractures are one of the most common fractures of the wrist and it occurs when falling on an outstretched hand. The most common type of fracture is a Colles’ fracture.

What are the symptoms of distal radius fracture?

Symptoms include swelling, pain in the wrist, and sometimes a visible deformity of the wrist. In some cases, it may be mistaken for a sprained wrist.

Dr. Vossoughi confirms the diagnosis by performing a physical examination and performing in-office X-rays. In some cases, only splint/cast treatment is indicated. In the case of displaced fractures, Dr. Vossoughi would perform the most advanced surgical techniques of open reduction internal fixation. In the case of severe injuries, external fixation may be indicated.

Scaphoid Fracture

What is a scaphoid fracture?

Scaphoid or boat-shaped bone is one of the eight bones in the wrist. It is usually caused by a fall onto an outstretched hand.

What are symptoms of a scaphoid fracture?

Many patients feel swelling and pain along the thumb side of the wrist. Symptoms may be mild and be mistaken for a sprained wrist. If not treated, arthritis, and chronic pain may develop.

Dr. Vossoughi would obtain in-office X-rays. Sometimes a CT scan or MRI would be indicated to provide an optimal view of anatomy to assist with diagnosis and treatment planning. The scaphoid bone has a unique and delicate blood supply. The treatment would be determined depending on where the fracture is located on the bone. The treatment options would vary between cast only or surgical stabilization of fracture, utilizing pins or screw. The treatment goal is to minimize pain, and risk for arthritis, and maximize the range of motion in the wrist.

Wrist Arthritis

Thumb basilar joint arthritis

Thumb basal joint arthritis is common arthritis found in the base of the thumb and feels also sometimes as wrist pain. It is most commonly due to osteoarthritis or degenerative arthritis which is a process of joint cartilage deterioration.

What are the signs and symptoms of thumb arthritis?

Pain in the base of the thumb when gripping, pinching, weakness, and swelling of the joint are common symptoms.

Dr. Vossoughi confirms the diagnosis by performing a physical examination and in-office x-rays. Treatment is based on the severity of arthritis. Alternatives for treatment include oral anti-inflammatory medications or steroid injections into the joint. In the case where conservative treatment fails, Dr. Vossoughi performs a  Ligament Reconstruction Tendon Interposition (LRTI) procedure by utilizing the most advanced techniques to reconstruct the joint. This procedure provides great pain relief for most patients.
